Customer Satisfaction Survey March 15 – 24

From March 15 – 24, 2017, Basics is conducting a shopper satisfaction survey. Customers on our email list will receive the survey in their inboxes by Wednesday, March 15th. If you are not on our email list, please stop by the Customer Service desk to pick up a paper copy! It will only take about 10 minutes of your time.

Your feedback will provide valuable information to our management and Board of Directors, helping us to improve store operations and to better serve you, our Owners and customers.

In appreciation for your participation, everyone who completes the survey will be entered into a drawing for Basics gift cards worth $10, $25, or $100 – 10 will win!

Your participation is voluntary but would be greatly appreciated. We are working with the Survey Research Center at the University of Wisconsin-River Falls to conduct this evaluation. The Survey Research Center’s participation will ensure an unbiased assessment of the data and your anonymity. At no time will any of the recorded responses be identified with individual customers. The last question on the survey asks for your contact information; this will be used only to contact drawing winners. Your survey responses will remain separate.

If there is more than one adult in your household, please have the adult who is the primary food shopper complete the survey.
